New Model 3350?

I saw an online ad for an aplification system using the ATR 3350 on a Native American Flute. It included a monoral plug and a 1/4" adaptor to be plugged into a Roland Micro Cube. The picture on ebay was just like the above description. When the ATR 3350 arrived it had a stereo plug and no 1/4" adaptor in the package. it would not work with my Micro Cube. What happened?

I just ordered this from Amazon and have the same problems: the 3.5mm plug is TRS (stereo), not TS (mono), and the 1/4" adapter shown in the Amazon photo and description are missing.
